Schedule
- PRN (as needed), remote position
- Minimum 4 shifts per month if needed
- Priority given to nights/weekends
- 2 holiday shifts per year
- 2 nights per quarter or 1 weekend per quarter if offered
Responsibilities
- Respond to inbound patient questions and escalations via phone or text.
- Follow standardized triage protocols (Schmitt-Thompson) and escalate appropriately, from after-hours clinic referrals to 911 emergencies.
- Document patient interactions and alert resolution in the Cadence platform; manage follow-up tasks in ZenDesk (training provided).
- Follow up on abnormal patient readings to gather more information and triage.
- Monitor vitals, symptoms, and labs to identify patients in need of intervention.
- Provide administrative support such as rescheduling patients or contacting those with lapses in monitoring.
- Deliver compassionate, high-quality patient interactions aligned with Cadence’s mission and partner health system goals.
Requirements
- Active RN compact multi-state license plus one additional state (CA, MI, IL, or AK); willingness to expand licensure as needed.
- 3+ years’ experience treating CHF, hypertension, and/or diabetes patients in inpatient or outpatient settings.
- Passion for patient care and digital healthcare innovation.
- Independent, adaptable operator comfortable in less structured environments.
Compensation
- $35–$40 per hour
- Additional $10/hour for night shifts
