Schedule
- Full-time, remote position
- Standard weekday schedule, with flexibility needed for outreach goals
Responsibilities
- Conduct high-volume outreach calls to patients and caregivers, educating them about available health services and scheduling appointments.
- Build trusting relationships with patients and caregivers, adapting communication styles to meet individual needs.
- Accurately track outreach and patient interactions in the CRM and electronic medical record (EMR).
- Support patients in navigating their healthcare journey, providing seamless handoffs to the clinical team.
- Collaborate with care teams to ensure smooth patient transitions and consistent engagement.
- Meet or exceed weekly outreach and enrollment goals.
- Provide feedback on outreach strategies to improve patient engagement and program success.
- Participate in virtual team meetings and remain self-motivated in a fast-paced remote environment.
Requirements
- 2+ years of customer service or sales experience; cold outreach experience preferred.
- 2+ years in healthcare outreach, community engagement, or related fields.
- Strong interpersonal skills with the ability to quickly build trust and rapport.
- Experience working with vulnerable populations, including seniors or individuals with behavioral health needs.
- Comfortable using G-Suite, video conferencing platforms, and EMR/CRM systems.
- Strong organizational skills with proven ability to manage priorities and meet outreach metrics.
- Knowledge of HIPAA requirements and ability to handle confidential information.
- Enthusiasm for working in an innovative, mission-driven startup.
