Schedule
- Remote, full-time position
- Monday through Friday, daytime hours (EST schedule)
- High-volume phone and virtual outreach
What You’ll Do
- Conduct outbound outreach calls to educate patients and caregivers on care services and benefits
- Help patients schedule and attend appointments while building trust and relationships
- Accurately document outreach and interactions in CRM and medical record systems
- Work closely with care teams to provide seamless handoffs and patient support
- Meet weekly outreach and enrollment goals in a metrics-driven environment
- Participate in virtual team meetings and contribute to improving outreach strategies
What You Need
- 2+ years in customer service or sales, with phone outreach experience preferred
- 2+ years in healthcare outreach, community engagement, or mental health support
- Strong ability to connect quickly with individuals, especially vulnerable populations
- Knowledge of healthcare or social services and an understanding of seniors’ needs
- Proficiency with G-Suite, CRM systems, and video conferencing
- Comfort working independently in a remote, fast-paced startup environment
- Adherence to HIPAA and patient confidentiality standards
