What You’ll Do
- Conduct high-volume outreach via phone to educate patients and caregivers on available services and schedule appointments.
- Build meaningful, trusting relationships with individuals facing behavioral health and social challenges.
- Track outreach, document member consent, and record notes accurately in CRM and electronic medical records.
- Collaborate with clinical care teams to ensure smooth handoffs and continuity of care.
- Meet monthly outreach and enrollment goals in a metrics-driven environment.
- Provide feedback on outreach strategies to improve patient engagement.
- Stay engaged in a dynamic, fully remote startup environment.
What You Bring
- 2+ years’ experience in customer service, sales, healthcare outreach, or community engagement (cold outreach experience preferred).
- Familiarity with healthcare, social services, or mental health support, especially for seniors.
- Strong interpersonal skills with patience, empathy, and cultural competency.
- Ability to quickly build trust with vulnerable populations.
- Tech-savvy with proficiency in G-Suite, CRM systems, and video conferencing platforms.
- Organized, goal-driven, and able to thrive in a fast-paced, evolving startup.
- Knowledge of HIPAA and confidentiality practices.
