Job Description
An employer in the Orange County area is seeking 2 Patient Care Coordinators. This is a remote position – client prefers if candidates sit in the Southern California area. The patient care coordinators will support two different groups: Transition of Care and General Care Management. The transition of care includes working with patients after they are discharged, assisting with follow up appointments, weekly check ins, and overall supporting a smooth transition once discharged from the hospital. The coordinator supporting general care management will help identify which group within the organization the patient should be directed to and assisting with all details included in that transfer. For both of these roles, the candidate must have very strong critical thinking skills, must be very organized and have great communication.

Skills and Requirements
2+ years of experience in a healthcare related coordinator role
Good understanding of medical terminology
Strong critical thinking skills
Experience working with any healthcare system null
