Job Description:
Baptist Health is looking for a Home Care Intake Clinical Specialist RN to join our team. This is a remote position that requires residency in KY and IN.
The Home Care Intake Clinical Specialist RN assess and evaluates patient referrals to determine and verify qualifying factors for admission to homecare including but not limited to appropriate need for skilled services, qualifying diagnosis, valid Face-to-Face elements and physician order.
Education
Associate’s degree: Nursing
RN/LPN licenses in the state where services are provided
If an additional RN licensure is required, the employee will show proof of
Endorsement within 6 months
Experience
Two or more years home health experience preferred
Prior discharge planning or intake experience preferred
Knowledge of PDGM and ICD 10 coding for home services preferred
