About the Role
As a Discharge Planner, you’ll support clients at the end of their treatment by developing thorough discharge plans and maintaining communication with referral providers such as hospitals, outpatient practices, schools, and community organizations. You’ll ensure clients and families are set up for success post-program by connecting them with appropriate aftercare resources while fostering trusted relationships with external providers.
What You’ll Do
- Provide treatment updates to referral sources and answer provider questions about clients
- Build collaborative relationships with external providers to ensure continuity of care
- Develop and coordinate individualized discharge plans for clients completing treatment
- Identify and refer clients to appropriate aftercare resources using Charlie Health’s nationwide provider database
- Follow up with clients and providers to confirm placements and smooth transitions
- Ensure all necessary clinical documentation is shared with referral providers
- Document case management contacts and collaborate with the treatment team on discharge updates
- Partner with internal Charlie Health teams (clinical, admissions, outreach) to coordinate care
- Adhere to policies, procedures, and performance goals
What You Need
- Bachelor’s degree in psychology, social work, health sciences, communications, or related field
- 2+ years of relevant work experience (case management, discharge planning, referral relations, admissions, or outreach)
- Strong interpersonal and relationship-building skills
- Results-oriented mindset with proven ability to meet metrics or performance targets
- Excellent written and verbal communication skills
- Extreme organization and attention to detail
- Authorized to work in the US with native or bilingual English proficiency
- Proficiency in Salesforce and Google Suite/MS Office
- Ability to thrive in a fast-paced environment and learn quickly
Compensation & Benefits
- Base salary: $52,500 – $60,000 per year (based on experience and location)
