Schedule

  • Full-time, remote
  • Must be able to work late afternoons and evenings
  • Not available to candidates in: Alaska, Maine, Washington DC, New Jersey, California, New York, Massachusetts, Connecticut, Colorado, Washington State, Oregon, or Minnesota

What You’ll Do

  • Serve as a key point of contact for clients, families, and referral sources
  • Provide timely updates to hospitals, clinicians, schools, and care partners
  • Create comprehensive, clinically appropriate discharge plans
  • Identify and secure aftercare services tailored to each client’s needs
  • Communicate referrals through provider-preferred channels and confirm placement
  • Maintain accurate case notes and collaborate with clinical and admissions teams
  • Meet defined service and performance goals

What You Need

  • Bachelor’s degree in health sciences, social work, psychology, communications, or related field
  • 2+ years experience in healthcare or behavioral health (case management, discharge planning, referrals, admissions, or outreach preferred)
  • Strong relationship-building and communication skills
  • Proven ability to meet detailed metrics and deadlines
  • Salesforce and Google Workspace/MS Office experience
  • Ability to thrive in a fast-paced, mission-driven environment
  • U.S. work authorization and fluent English

Benefits

  • Base salary: $52,500–$60,000
