Schedule

  • Full-time, 100% remote position
  • Shifts may include afternoons and evenings (depending on candidate availability)

What You’ll Do

  • Provide consistent, high-quality treatment updates to referral providers (hospitals, schools, clinics, government agencies, etc.) regarding client progress
  • Develop discharge plans for clients completing treatment, identifying aftercare resources and facilitating a smooth transition
  • Maintain professional, trust-based communication with referral providers, answering questions about referred clients
  • Use nationwide provider databases to locate appropriate referral options and manage referrals via providers’ preferred channels
  • Follow up with clients and providers to confirm placements and ensure care continuity
  • Document case management contacts, collaborate with therapists and clinical teams to keep discharge plans updated
  • Partner with internal Charlie Health teams (clinical, admissions, outreach, partnerships) to ensure coordination and success
  • Meet organizational performance metrics while adhering to all policies and procedures

What You Need

  • Bachelor’s degree in health sciences, communications, psychology, social work, or related field
  • 2+ years of relevant work experience (e.g., case management, discharge planning, referral relations, admissions, healthcare outreach)
  • Strong interpersonal, relationship-building, and listening skills
  • Results-oriented mindset, with proven experience meeting concrete targets (KPIs)
  • Excellent written and verbal communication skills
  • Extreme organization and attention to detail
  • Authorization to work in the U.S. and native/bilingual English fluency
  • Ability to thrive in a fast-paced environment and learn quickly
  • Proficiency in Salesforce and Google Suite/MS Office

Compensation & Benefits

  • Base salary: $52,500–$60,000/year (adjusted based on experience and location)
  • Total compensation package may also include incentive pay, discretionary bonuses, and additional Charlie Health-sponsored benefits
