Cityblock Health is the first tech-driven provider for communities with complex needsbringing better care to where it’s needed most, block by block. Founded in 2017 on the premise that health is local and based in Brooklyn, we are backed by Alphabet’s Sidewalk Labs along with some of the top healthcare investors in the country.
Our mission is to improve the health of underserved communities. Importantly, our solutions are designed specifically for Medicaid and lower-income Medicare beneficiaries, and we meet our members where they are, bringing care into the home and neighborhoods through our community-based care teams and Virtual Care offerings.
In close collaboration with community-based organizations, local providers, and leading health plans, we are reorganizing the health system to focus on what matters to our members. Equipped with world-class, custom care delivery technology, we deliver personalized primary care, behavioral health, and social services to deliver a radically better experience of care for every member and community we serve.

About the Role:
- Under the direction of the Manager of Central Scheduling and the Scheduling Lead, the Central Scheduler will outreach and engage Cityblock members to schedule appointments with our care teams.
- Weekly tasks include:
- Schedule appointments for new and existing members with their Cityblock care team, including appointments with a primary care provider, nurse, or other care team member
- Outreach and engage new Cityblock members by introducing them to Cityblock’s services, starting with a welcome appointment with their care team.
- Answer appointment related questions from members and our internal teams
- Document all interactions with members in Commons and/or EHR
- Confirm appointments with members and ensure that they are prepared and informed of what to expect during their visit. This includes reviewing the technical requirements with members for video visits
- Resolve any appointment conflicts for our members by modifying appointments in the EHR, Commons, Traumasoft and other scheduling systems
Requirements for the Role:
- Prior customer service experience, including welcoming individuals to new programs
- Excellent written and verbal communication skills
- Strong organizational skills and able to manage a pre populated list of members to outreach and schedule appointments is able to track the status of an appointment
- Computer savvy and can easily navigate multiple systems.
- Able to work effectively and collaboratively, with a sharp sense of member experience and scheduling priorities
- Able to meet productivity targets while working remotely
- Prior scheduling experience, preferably using EHRs
