Reporting to a Case Management Manager, Care Coordinators work closely with Case Managers for reporting, care coordination phone calls, and other administrative duties. Care Coordinators increase efficiency of operations by providing general customer support duties and by supporting the nursing and operations staff in a team environment. Care Coordinators are the “right hand person” to the Case Managers and an integral part of our Case Management Admin Team!
Work Specifics: Non-Exempt, Monday-Friday, 8 am – 5 pm Central Time – Remote
This position pays $18-$21/hr depending on experience, education and location.

Responsibilities:
- Provide accurate information to callers based on customer requests; triaging telephone calls to direct the caller for clinical consultation and for case managers
- Enter new claims data into the claims management system accurately; maintain data integrity in scanning, uploading, indexing and data entry
- Support clinical staff through the completion of components of case management, including but not limited to appointment scheduling, diagnostic test scheduling, requesting medical records, faxing materials, mailing/e-mailing already identified education materials, scheduling delivery of already negotiated and approved DME, and facilitating claims adjudication
- Professional interaction with Case Managers, Insurance Adjusters and other medical professionals
- Assist with process, product and technology improvement projects
- Assist with quality improvement projects as needed
- Perform inter-departmental administrative support as needed
- Provide support to the CM Intake Coordinator as needed
- Other duties as assigned
Requirements
- Experience in a medical office environment preferred
- AS/AA degree or two (2) years of college preferred
- Professional demeanor with excellent written and oral communication skills
- Strong organization skills
- Must be computer literate with a high comfort level with computer programs (MS Office, Email and Internet)
- High comfort level with office equipment: fax machines, copy machines, and scanners
- Must be Punctual and Dependable
- Able to maintain focus and positive attitude in a fast-paced office
- Ability to work with minimal supervision
- Ability to meet deadlines in a high pressure, time sensitive environment
- Must be able to lift up to 20 lbs.
