Schedule

  • Remote role within the U.S.
  • Monday–Friday, 1:00 PM – 9:00 PM ET (10 AM – 6 PM PT).

What You’ll Do

  • Provide navigation support for patients, helping them find in-network providers and access social services (housing, transportation, WIC, SNAP).
  • Monitor and respond to calls on the member support line.
  • Complete eligibility checks, manage patient offboarding, and handle scheduling (prioritization, reminders, provider optimization).
  • Manage referrals to health plan Case Management teams.
  • Collaborate with Market Operations Lead to improve workflows and efficiency.
  • Perform other assigned operational and administrative tasks.

What You Need

  • 4+ years of healthcare administration experience, ideally in virtual care.
  • Familiarity with SDOH (social determinants of health) platforms and referrals.
  • Strong attention to detail and organizational skills.
  • Ability to work cross-functionally with clinical teams.
  • Patient-first mindset and passion for maternal and child health.
  • Comfort with ambiguity and fast-moving environments.
