Schedule

  • Full-time, remote
  • May require up to four in-person visits per year to Columbia, MD for company events
  • Priority hiring for candidates in Florida, Illinois, and Kentucky
  • Requires reliable technology and ongoing collaboration with a remote clinical team

Responsibilities

  • Conduct immediate outreach to members with out-of-range vital signs or device alerts
  • Assess symptoms, identify risks, and provide guidance on when to seek urgent or emergency care
  • Educate members on chronic conditions, device ranges, and available support resources
  • Assist members with mobile care management devices and troubleshoot basic issues
  • Maximize member use of available programs and resources
  • Document all interactions accurately in Vheda’s platform
  • Support health coaches, tech support, and customer service teams
  • Collaborate with the clinical team to ensure cohesive member care
  • Contribute to closing gaps in care and supporting care plan adherence

Requirements

  • Fluent in Spanish
  • Current and active RN license
  • 1–3 years of experience in a critical care setting
  • Strong understanding of chronic disease management (Diabetes, Hypertension, CHF, Asthma, COPD, Weight Management, Behavioral Health)
  • Proficient with web-based systems and remote workflows
  • Excellent communication skills with compassion, empathy, and cultural awareness
  • Ability to work independently and collaboratively in a remote environment

Preferred

  • Emergency room experience
  • Experience working with diverse populations
  • Knowledge of Medicaid and/or Medicare
  • Prior telephonic patient outreach
  • Motivational Interviewing familiarity
  • Experience in care coordination or case management
  • Knowledge of HEDIS and/or NCQA standards
