Schedule
- Full-time, remote (U.S. based)
- 8:30 AM – 5:00 PM CST
- Must be bilingual in English & Spanish
Responsibilities
- Answer inbound calls to assist members with assessments and recommend services
- Make outbound calls to follow up with members and families
- Guide members in accessing home care, transportation, meals, errands, pest control, home modifications, and social support services
- Coordinate and schedule services based on member needs
- Ensure accurate documentation and follow-up for each interaction
- Consistently meet call metrics, CSAT targets, and attendance adherence
Requirements
- Bilingual: fluent in English & Spanish (spoken and written)
- High school diploma (preferred)
- Experience in public/community health, care coordination, or social services (preferred)
- Strong computer proficiency and organizational skills
- Excellent written, verbal, and interpersonal communication
- Proven experience with inbound call handling
- Ability to work in a fast-paced, evolving environment
Physical/Technical Requirements
- Reliable internet connection and distraction-free home office
- Ability to remain at workstation throughout the workday
- Proficiency with VoIP systems, computers, and online platforms
Compensation
- $19.23/hour
