Schedule
- Full-time, remote position
- Standard business hours, with some local travel within service area as needed
Responsibilities
- Conduct telephonic outreach using motivational interviewing techniques
- Educate members on behavioral health conditions, treatments, and resources
- Coordinate behavioral health and substance use disorder treatment and community referrals
- Triage referrals and calls from providers, members, and internal teams
- Perform post-discharge follow-up calls and screenings
- Act as liaison between members, providers, and external agencies to ensure timely care
- Document all coordination activities in the medical management system (e.g., Jiva)
- Support BH Case Management and Utilization Management teams with administrative needs
- Identify workflow/process improvements and contribute to departmental goals
Requirements
- High school diploma with significant behavioral health coordination experience, OR Associate’s degree in healthcare/related field (required), OR Bachelor’s in psychology/social work (preferred)
- Experience in customer service, call center, or healthcare office setting
- Strong motivational interviewing, communication, and organizational skills
- Ability to manage multiple tasks in a fast-paced environment
- Knowledge of behavioral health and substance use disorder services preferred
- Proficiency with Microsoft Office (Teams, Outlook, Word, Excel, PowerPoint)
- Bilingual/multilingual abilities a plus
