What you will do:
As the Afterhours Referral Specialist, you will be responsible for monitoring faxes, multiple referral sites and email, gathering and entering information related to the evaluation and admission of patients, verifying patient eligibility/benefits, coordinating non – clinical aspects of an admission and scheduling visits. This position supports our Hospice care centers
Essential Functions:
- Processes after-hours tasks that can be accomplished by a non-clinical person such as visit assignments and changes, faxing documents, medical supply request and assisting with DME (Durable Medical Equipment) needs.
- Serves as a first line of customer service for internal and external customers.
- Utilizes computer to monitor faxes and referrals.
- Receives and documents patient referrals, ensuring compliance with applicable coverage criteria.
- Gathers and enters appropriate information into the EMR system to schedule an evaluation visit.
- Coordinates the evaluation/admission process and may assign field staff to conduct initial assessment.
- Communicates pertinent patient information to the care center team.
- Conducts insurance verification and verifies benefit period and payer authorization as needed.
- Communicates and follows up with Business Development regarding pending admissions as needed or as directed by Director of Operations.
- Receives and coordinates requests for medication and supplies including durable medical equipment
- Faxes documents from Focura/HCHB as requested.
- Schedules visits using the electronic medical record.
- Obtains information from agencies when necessary to assist with entering referral information and assigning start of care visits.
- Serves as a patient care advocate.
- Complies with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.

Qualifications:
Required:
- High School diploma or GED equivalent.
- Must be familiar with functions of the computer, such as usernames and password; internet; email; navigation of computer desktop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.
