What You’ll Do
- Call eligible patients and explain care program benefits in a clear, patient-friendly way
- Support the clinical team with patient communication and coordination
- Confirm eligibility and verify insurance coverage
- Maintain accurate records of patient interactions and documentation for the patient chart
- Communicate professionally and empathetically to build trust and encourage participation
- Coordinate with internal teams and healthcare providers for smooth handoffs
- Meet or exceed enrollment and performance targets
What You Need
- Healthcare experience preferred
- Comfortable with basic computer applications and data entry
- Strong problem-solving skills and ability to juggle multiple priorities
- Excellent verbal and written communication skills
- Collaborative, positive team mindset
- Ability to work independently in a remote environment
- Able to pass a background check and drug screening
- Experience with multiple EMR systems and payer portals
