This is the “keep the doors open” intake role. You’re the steady hand making sure referrals get processed, patients get onboarded, and nothing stalls out just because it’s the weekend. If you like being the one who keeps the machine moving when everyone else is off, this is that job.

Schedule
⦁ Full-time (40 hours/week)
⦁ Thursday–Monday, 8:30 AM–5:00 PM (weekend coverage required)
⦁ Fully remote
⦁ Pre-employment drug screen required (includes medical and recreational marijuana)

What You’ll Do
⦁ Process home health referrals accurately and quickly, including complete data entry from referral packets
⦁ Complete basic diagnosis coding based on referral info
⦁ Coordinate patient care using the Welcome Call Script
⦁ Identify referrals needing clinical documentation and route to Clinical Intake/RN when needed
⦁ Support Starts of Care (SOC) and Resumption of Care (ROC)
⦁ Serve as liaison between the company, patients/caregivers, and business partners
⦁ Track and follow up with patients pending start of care
⦁ Track and follow up with patients in hospitals or facilities
⦁ Explain company programs clearly to patients and referral sources
⦁ Attach/scan referral packets and patient documents
⦁ Coordinate with referral sources and the sales team regarding new referrals

What You Need
⦁ Associate’s degree preferred
⦁ Proficiency with Microsoft Office, CRM tools, and Home Care Home Base (HCHB)
⦁ 1+ year home health experience preferred
⦁ 1+ year customer service experience
⦁ Strong critical thinking (they’re explicit about this being required)
