This is a front-door role. You’re the person who catches the referral, makes sure nothing gets dropped, and keeps patients moving toward Start of Care without delays. If you’re sharp with details, calm on the phone, and can juggle follow-ups like it’s a sport, this is your lane.

Schedule
⦁ Full-time (40 hours/week)
⦁ Monday–Friday, 8:00 AM–5:00 PM (fully remote)
⦁ Pre-employment drug screen required (includes medical and recreational marijuana)
What You’ll Do
⦁ Receive and process home health referrals accurately and fast, including complete data entry from the referral packet
⦁ Do basic diagnosis coding based on referral information
⦁ Coordinate care using the Welcome Call Script (patient outreach and intake confirmation)
⦁ Identify referrals that require additional clinical documentation and route to Clinical Intake/RN when needed
⦁ Manage Starts of Care (SOC) and Resumption of Care (ROC) processes
⦁ Serve as liaison between the company, patients/caregivers, and business partners
⦁ Track and follow up with patients pending start of care
⦁ Track and follow up with patients currently in hospitals or facilities
⦁ Explain company programs clearly to patients and referral sources
⦁ Attach/scan referral packets and patient documents
⦁ Communicate with referral sources and the sales team on new referrals
What You Need
⦁ Associate’s degree preferred
⦁ Proficiency with Microsoft Office, CRM software, and Home Care Home Base (HCHB)
⦁ 1+ year home health experience preferred
⦁ 1+ year customer service experience
⦁ Strong critical thinking (they call this out as required)
⦁ Comfortable handling multiple follow-ups while staying accurate and organized
