If you’re an LPN who likes fast pace, clean workflows, and talking to patients all day (without the floor chaos), this is a solid lane. You’ll complete intake assessments for newly enrolled chronic care patients and help coordinate the resources that keep them stable.

Schedule
- Full-time, 100% remote
- Monday–Friday, no weekends
- Rotational on-call: about once per year on average
- Shift options (CST):
- 8:00 AM–4:30 PM
- 9:00 AM–5:30 PM
- Most of your shift is on the phone
- Remote requirements: high-speed Wi-Fi + HIPAA-compliant home office/workspace
What You’ll Do
- Accept warm transfers from the Patient Enrollment team for newly enrolled patients
- Conduct preliminary health assessments and clarify patient needs through open dialogue
- Identify and coordinate community resources to support care plans
- Provide patient education and improve health literacy around chronic conditions
- Support medication management (adherence concerns, refill coordination, potential issues)
- Help coordinate timely services like Home Health, DME, Home Infusion, and other critical needs
- Take thorough notes in real-time and keep documentation accurate
What You Need
- Active Compact/Multi-State LPN (LVN) license
- At least 3 years of direct, patient-facing experience
- Strong written/verbal communication and a calm, confident phone presence
- Comfort working quickly in a fast-paced environment with high call volume
- Technical aptitude with Microsoft Office Suite
- Strong attention to detail and strong documentation habits

Pay
- Starts at $21/hr
- Ability to earn up to $28/hr based on production
