If you’re an LPN who likes fast-paced phone work, enjoys meeting new patients, and can spot gaps in care quickly, this is a strong fit. You’ll complete initial assessments for newly enrolled chronic care patients and help coordinate resources, meds, and services that keep them stable at home.

Schedule
- Full-time, 100% remote
- Monday–Friday (no weekends)
- Rotational on-call: about once per year on average
- Shift options (CST):
- 8:00am–4:30pm
- 9:00am–5:30pm
What You’ll Do
- Take warm transfers from the Patient Enrollment team and complete preliminary health assessments for newly enrolled patients
- Answer patient questions and create an open, supportive dialogue to uncover needs
- Identify and coordinate helpful community resources for patient care
- Provide patient education and improve health literacy around chronic condition management
- Support medication management, including adherence concerns, identifying potential issues, and coordinating refills
- Help coordinate timely services like Home Health, DME, Home Infusion, and other critical needs
- Spend most of your shift on the phone while documenting thoroughly and accurately
What You Need
- Active Compact/Multi-State LPN/LVN license (required)
- At least 3 years of direct patient-facing experience (required)
- Strong written and verbal communication skills
- Solid technical aptitude (Microsoft Office Suite)
- Ability to adapt quickly in a fast-paced environment
- HIPAA-compliant home workspace
- Active high-speed Wi-Fi

Pay
- Starts at $21/hr
- Can earn up to $28/hr based on production
