About Pair Team
Pair Team is a tech-enabled medical group delivering whole-person care for Medicaid and Medicare populations by integrating clinical, behavioral, and social supports. They partner with community organizations and clinics, and use AI and automation to reduce admin burden so clinicians can focus on care. They support Enhanced Care Management through CalAIM in California and aim to scale equitable, community-driven healthcare nationally.

Schedule
• Full-time, 100% remote (United States)
• Work from home with equipment provided
• Must be comfortable working Pacific Time hours
• Collaborative, interdisciplinary team environment (clinical, care management, product, operations)

What You’ll Do
• Provide best-in-class virtual preventive care for underserved patients using internal care delivery technology and clinical judgment
• Develop and refine clinical programs that address barriers and gaps in care holistically
• Collaborate with the product team to improve the platform and workflows for care delivery
• Partner closely with multidisciplinary teams, clinics, and community organizations supporting complex patients
• Build trust-based relationships with patients who may be hesitant or distrustful of the healthcare system
• Support care coordination within an interdisciplinary Enhanced Care Management model

What You Need
• 2+ years clinical experience serving patients with complex social and healthcare needs
• 1+ year primary care experience
• Board certified or board eligible in Family Medicine
• Comfortable caring for patients across the lifespan (pediatrics to geriatrics)
• Experience with, or strong interest in, working with multidisciplinary care teams
• Tech-savvy and comfortable in digital-first care environments (telemedicine and multiple platforms)
• Experience in Medicaid and/or FQHC settings (or strong alignment with that environment)
• Bilingual English and Spanish
• Active Nurse Practitioner license in California
• Active DEA license preferred
• Startup experience is a plus
