- Remote 1099 contractor
- Full-time or part-time available
- Compensation:
- Full-time 1099: $6.8K to $7.4K per month
- Part-time 1099 (20+ hours/week): $3.6K to $4.4K per month
What You’ll Do
- Build trusting, empathetic relationships with Medicare patients
- Guide patients through complex health concerns and identify priority needs
- Create care plans that address social determinants of health such as food, transportation, and home support
- Navigate patients through their care journey and maintain a cohesive care continuum
- Use Solace tools, systems, and partners while contributing your own expertise
- Help shape the systems and processes used to support Medicare patients
What You Need
- 3+ years of experience in care management, patient advocacy, or healthcare navigation
- Strong understanding of Social Determinants of Health
- Deep empathy and a commitment to advocating for vulnerable populations
- Strong clinical judgment and organizational skills
- Ability to multitask and communicate clearly under pressure
- Comfortable adopting new technology and systems
- Action-oriented mindset and willingness to provide direct, constructive feedback
- Must reside in the United States
