Schedule

Responsibilities
- Triage and delegate care needs using clinical knowledge and critical thinking
- Contact members with out-of-range vital signs or device readings
- Assess urgency, escalate care appropriately, and document all conversations
- Provide telephonic consultation to assigned cohorts to determine disposition
- Reengage non-compliant members with empathy and clear communication
- Collaborate with clinical services, health coaches, and member support teams
- Maintain strong working relationships across the care team
- Facilitate or delegate patient education aligned with standards of care
- Assist members with mobile care management devices
- Maximize member access to available health resources
- Support HEDIS and/or NCQA-related activities
- Conduct care team outreach with providers
- Maintain knowledge of Medicaid and/or Medicare guidelines
Requirements
- Proficient with technology and web-based applications
- Ability to communicate clearly with members, care teams, and support persons
- Strong interpersonal, organizational, and communication skills
- Demonstrated compassion, empathy, and cultural awareness
- Ability to work effectively with diverse populations
- Experience working collaboratively in a remote, fast-paced environment
