- Requires reliable computer skills and comfort working in web-based platforms

Responsibilities
- Triage and assess member needs using critical thinking and clinical guidance
- Contact members with out-of-range vital signs or device alerts
- Determine urgency, provide next-step guidance, and escalate care when appropriate
- Offer telephonic support for assigned member cohorts
- Re-engage non-compliant members and support care plan adherence
- Document all interactions accurately
- Collaborate with clinical services, health coaches, and support teams
- Educate members using appropriate modalities and approved standards of care
- Assist members with remote patient monitoring devices
- Maintain working knowledge of chronic conditions (Diabetes, Hypertension, CHF, Asthma, COPD, Weight Management, Behavioral Health)
- Support HEDIS/NCQA-related communications
- Participate in care team outreach with providers
Requirements
- Strong proficiency with computers and web-based tools
- Excellent communication skills with members and care teams
- Ability to demonstrate empathy, compassion, and cultural awareness
- Strong organizational and interpersonal skills
- Comfortable working with diverse populations
- Understanding of Medicaid/Medicare preferred
