Schedule
- Remote, U.S. based
- 1099 independent contractor role
- Choose part-time (20+ hrs/week) or full-time
Compensation
- Full-time: $6.8K – $7.4K/month
- Part-time: $3.6K – $4.4K/month
What You’ll Do
- Support Medicare patients through complex medical decisions and health challenges
- Build strong relationships grounded in listening and empathic care
- Create holistic care plans addressing clinical needs & social determinants (transportation, food access, home support, etc.)
- Ensure continuity of care and help patients navigate barriers
- Use Solace platforms and tools to document, coordinate, and communicate effectively
- Act as a trusted guide and fierce advocate throughout each patient’s journey
What You Need
- Active RN license in good standing
- Experience working with diverse patient populations
- Deep understanding of Social Determinants of Health
- Strong clinical judgment + excellent communication and documentation skills
- Technical confidence — comfortable learning software and systems quickly
- Action-oriented mindset with high empathy and resilience
- U.S. residency required
