Schedule
- Remote, full-time overnight role
- Two schedule options available:
- Thursday, Friday, Saturday, and every other Wednesday, 7 PM–7 AM EST
- Sunday, Monday, Tuesday, and every other Wednesday, 7 PM–7 AM EST
What You’ll Do
- Monitor patient vitals, labs, and symptoms in collaboration with RNs and MDs.
- Manage treatment plans and medications, with emphasis on guideline-directed therapy for CHF, T2D, and hypertension.
- Triage and respond to patient escalations, including abnormal vitals or urgent concerns.
- Conduct virtual check-ins with patients to optimize adherence and review labs.
- Support both primary care and cardiology practices through virtual care delivery.
- Document patient interactions and collaborate with care teams for seamless follow-up.
- Uphold Cadence’s mission by delivering exceptional patient experiences.
What You Need
- 5+ years of experience treating chronic conditions (CHF, hypertension, T2D) in an inpatient or outpatient setting.
- Active Compact Multi-State RN license.
- Master’s Degree as a Nurse Practitioner.
- Active Nurse Practitioner (NP) licenses in TX, CA, NC, IL, and NY required; AL, TN, OK, and WA strongly preferred.
- ANCC or AANP certification.
- Experience managing CHF with guideline-directed therapy (required).
- Ability to work independently, problem-solve, and communicate effectively in a fast-paced virtual environment.
- Prior experience with remote patient monitoring or high-growth healthcare startups preferred.
