Schedule
- Full-time, remote
- Monday–Friday, 8 AM–5 PM (Pacific, Mountain, or Central time zones)
- Stable, high-speed internet required (speed test before interview)
What You’ll Do
- Monitor patient vitals, symptoms, and labs alongside RNs and MDs
- Manage treatment plans and medications, including GDMT optimization for CHF, T2D, and hypertension
- Respond to patient escalations flagged through remote patient monitoring
- Lead regular virtual check-ins with patients for labs, adherence, and progress reviews
- Support both primary care and cardiology offices with remote care delivery
- Contribute to scaling Cadence’s programs and culture as we grow nationwide
What You Need
- 5+ years treating chronic disease patients (outpatient or inpatient)
- Active multi-state compact RN license
- Nurse Practitioner license in TX and one or more of the following states: AL, AR, CT, FL, IL, IN, KY, MI, MS, NC, NY, OK, PA, TN, VA, VT, WI
- ANCC or AANP certification
- Master’s degree as a Nurse Practitioner, with willingness to expand licensure as needed
- Experience with CHF management using GDMT
- Strong communication, problem-solving, and independent decision-making skills
- Preferred: experience in CHF/T2D clinics, with remote monitoring tech, or in a startup environment
Pay
- $125,000–$150,000 annual base salary
- Quarterly incentives available
