Schedule
- Fully remote 1099 contract role
- Full-Time: $6,800 – $7,400/month
- Part-Time (20+ hours/week): $3,600 – $4,400/month
- Flexible schedule with patient-facing responsibilities
What You’ll Do
- Build strong, trust-based relationships with Medicare patients through empathy and active listening
- Identify and prioritize patient needs, ensuring continuity of care
- Develop care plans that holistically address Social Determinants of Health (food, transportation, home support, etc.)
- Guide patients through complex healthcare systems to achieve their wellness goals
- Provide expertise and hands-on solutions while leveraging Solace’s systems, technology, and partnerships
- Contribute to shaping future patient support models by offering feedback and innovation
What You Need
- 3+ years’ experience in care management, patient advocacy, or healthcare navigation
- Strong knowledge of Social Determinants of Health and experience supporting diverse populations
- Deep empathy, advocacy skills, and clinical knowledge
- Excellent organizational and communication skills under pressure
- Technical proficiency with healthcare tools and systems
- U.S.-based residency required
