Schedule & Compensation
- 1099 Contract – Remote, U.S.-based only
- Full-Time: $6,800 – $7,400/month
- Part-Time (20+ hours/week): $3,600 – $4,400/month
- Flexible hours with fully remote work environment
What You’ll Do
- Build trusting, empathetic relationships with Medicare patients
- Guide patients through care planning and health system navigation
- Address Social Determinants of Health such as food, housing, and transportation
- Create patient care plans that support long-term wellness goals
- Use Solace’s proprietary tools and systems to streamline care delivery
- Advocate for patients and remove barriers standing in the way of their care
What You Need
- 3+ years of experience in care management, healthcare navigation, or patient advocacy
- Strong knowledge of Social Determinants of Health (SDOH)
- Clinical skills with proven organizational and multitasking abilities
- Comfort with technology and new systems/software
- Compassion, empathy, and an action-driven approach to patient support
- Must be based in the United States
