- Full-time, remote role

What You’ll Do
- Provide treatment updates to referral providers and answer client-related questions
- Develop comprehensive discharge plans to ensure smooth client transitions
- Identify appropriate aftercare resources and verify placements
- Maintain accurate progress notes and update internal systems
- Build strong relationships with referral sources, external providers, and clients
- Collaborate closely with clinical, admissions, and outreach teams
What You Need
- Bachelor’s degree in health sciences, communications, psychology, social work, or related field
- 2+ years of experience in healthcare (discharge planning, case management, outreach, or similar)
- Strong interpersonal and relationship-building skills
- Results-driven with experience working toward performance targets
- Excellent communication, organization, and attention to detail
- Proficiency in Salesforce, Google Suite, and MS Office
- Authorized to work in the U.S.
