Help patients stay connected to preventive and wellness care. Aledade is seeking a Scheduling Outreach Specialist to conduct outreach calls, educate patients, and coordinate appointments that improve continuity of care.

Schedule
- Full-time, Monday–Friday 8:30am–5:00pm (local time)
- Occasional afternoon, evening, or Saturday morning shifts as needed
- 100% remote
What You’ll Do
- Conduct telephonic outreach to patients for wellness and preventive care scheduling
- Follow standard messaging and provide excellent service to encourage appointments
- Apply motivational interviewing to address barriers and support patient needs
- Educate patients on available services and redirect calls when necessary
- Track outreach and outcomes in Aledade systems (apps, Google Drive, EHR platforms)
- Identify outreach trends, report findings, and suggest creative improvements
- Collaborate with market teams and practice leadership for coordinated care
What You Need
- High school diploma required
- 2+ years of healthcare experience
- 1+ year of telephonic customer service or call center experience
- Knowledge of call center operations and healthcare scheduling workflows
- Proficiency with Microsoft and Google applications; familiarity with EHR systems
- Strong communication, organization, and motivational skills
- Access to reliable high-speed internet
- Bilingual preferred
