Charlie Health is seeking a Discharge Planner to support client care transitions and strengthen relationships with referral sources. In this role, you’ll ensure clients and families leave Charlie Health with a clear, clinically appropriate plan and that providers remain updated throughout treatment.

Responsibilities
- Treatment Updates: Provide professional, timely updates to referral providers and answer questions about referred clients.
- Discharge Planning: Develop thorough discharge plans tailored to client needs, connecting them with appropriate aftercare resources.
- Client & Provider Coordination: Work directly with clients, families, and external providers to ensure smooth transitions post-program.
- Database Management: Maintain and utilize Charlie Health’s provider database to identify referral options and confirm placements.
- Follow-Up & Verification: Confirm referrals with clients and external providers, ensuring all clinical materials are shared appropriately.
- Documentation: Maintain accurate case notes and communicate updates to therapists, case managers, and the treatment team.
- Cross-Functional Collaboration: Partner with internal admissions, clinical, and outreach teams to align discharge planning with broader care goals.
- Performance Goals: Meet established metrics for timeliness, accuracy, and client satisfaction.
Qualifications
- Bachelor’s degree in psychology, health sciences, communications, social work, or related field
- 2+ years of healthcare experience (case management, discharge planning, referral relations, admissions, or outreach)
- Strong communication and relationship-building skills
- Results-oriented with experience meeting performance targets
- Highly organized with strong attention to detail
- Proficient in Salesforce, Google Suite, and MS Office
- Authorized to work in the United States, fluent in English (Spanish bilingual a plus)

Compensation & Benefits
- Base Salary: $52,500 – $60,000 annually
- Total Compensation: May include discretionary bonuses and long-term incentive packages
