Full-time, remote (United States). Some late afternoon and evening availability required.
What You’ll Do
- Support clients and referral sources with consistent communication and treatment updates
- Develop discharge plans to ensure smooth, clinically appropriate transitions after treatment
- Make accurate referrals using provider databases and maintain follow-up to verify placement
What You Need
- Bachelor’s degree in health sciences, psychology, social work, or related field
- At least 2 years’ experience in healthcare, case management, or discharge planning
- Strong communication, relationship-building, and organizational skills
