Schedule
- Full-time, remote position.
- Must be authorized to work in the U.S.
- Native or bilingual English required; Spanish proficiency is a plus.
Responsibilities
- Provide consistent, high-quality treatment updates to referral providers (hospitals, outpatient practices, schools, government orgs).
- Build trusting relationships with referral sources through professional, collaborative communication.
- Develop comprehensive discharge plans for clients completing treatment, ensuring smooth transitions to aftercare.
- Identify and connect clients to appropriate aftercare providers using a nationwide provider database.
- Communicate with providers through preferred channels and follow up to confirm placements.
- Document case management contacts, update progress notes, and communicate with treatment teams.
- Partner with internal teams (clinical, admissions, outreach) to ensure discharge goals and referral needs are met.
- Adhere to compliance requirements, policies, and performance metrics.
Requirements
- Bachelor’s degree in health sciences, psychology, social work, communications, or related field.
- 2+ years of relevant work experience (case management, discharge planning, referral relations, admissions, outreach).
- Strong interpersonal, relationship-building, and listening skills.
- Results-oriented with experience meeting metrics and goals.
- Excellent verbal and written communication skills.
- Highly organized with strong attention to detail.
- Ability to thrive in a fast-paced environment.
- Proficiency in Salesforce and Google Suite/MS Office.
Compensation & Benefits
- Base salary: $52,500–$60,000 annually (depending on location and experience).
- Additional compensation may include incentive pay, discretionary bonuses, and other benefits.
- Comprehensive benefits package for full-time employees, including medical, dental, vision, life insurance, paid leave, and 401(k).
