Schedule
- Temporary, full-time role through December 31, 2025
- Remote position with completion bonus included
What You’ll Do
- Provide direct care navigation services to a patient caseload.
- Assess social, economic, and environmental factors impacting patient health.
- Advocate for patients by connecting them with financial, housing, and community resources.
- Build trust with patients and caregivers, motivating them to stay engaged in care.
- Facilitate communication between healthcare providers and community organizations.
- Maintain accurate documentation of patient interactions and outcomes.
- Conduct outbound calls to schedule patients as needed.
What You Need
- 3+ years of experience in care navigation, social work, or community outreach within healthcare.
- Excellent communication skills and ability to manage multiple communication channels.
- Highly organized, adaptable, and solution-oriented.
- Fluency in Spanish preferred but not required.
- Startup or on-demand service experience a plus.
