Schedule
- Full-time, Friday–Tuesday (off Wednesday & Thursday)
- Must be able to work PST (West Coast) hours
- Remote, U.S.-based (travel to clinics and partners as needed)
- Training: first 30 days Monday–Friday before transitioning to weekend schedule
What You’ll Do
- Identify, outreach, and enroll individuals living with Serious Mental Illness, Substance Use Disorder, homelessness, or high medical needs.
- Educate community partners and members about Enhanced Care Management services.
- Build trust with patients and help them navigate the enrollment process.
- Manage a high volume of outreach via phone, text, email, and in-person community engagement.
- Document all outreach activity and provide funnel updates including delays or blockers.
- Collaborate with internal care teams and external partners to improve communication and outcomes.
- Meet enrollment targets to support Pair Team’s mission of improving access for underserved communities.
What You Need
- Must reside in the United States with a HIPAA-compliant workspace and reliable internet.
- Bilingual English/Spanish (fluent in reading, writing, speaking, and listening).
- 1+ year of experience in a metric or quota-driven role.
- Strong communication skills and ability to engage vulnerable populations.
- Technical proficiency with multiple platforms (Outlook, Teams/Zoom, Excel, texting platforms).
- Valid driver’s license and auto liability insurance.
Preferred
- Experience with high-volume outreach or engagement campaigns.
- Background in healthcare, social services, or community work.
- Excellent listening skills and the ability to read people and adapt communication style.
