About the Role
As a Continuing Care Coordinator, you’ll manage communications with referral sources such as hospitals, outpatient practices, schools, and community organizations. You’ll provide treatment updates to referring providers and ensure clients have a comprehensive discharge plan when they complete care. This role is both client- and provider-facing, requiring strong relationship-building skills, attention to detail, and a commitment to supporting successful care transitions.
What You’ll Do
- Provide consistent treatment updates to referral sources and answer questions about referred clients
- Build trusting relationships with external providers, ensuring smooth collaboration
- Develop discharge plans that connect clients and families to appropriate aftercare resources
- Use and maintain a nationwide provider database to identify referral options
- Follow up with clients and providers to verify placement and ensure continuity of care
- Document all case management contacts in progress notes and communicate updates with the treatment team
- Partner with Charlie Health teams (clinical, admissions, outreach) to coordinate care effectively
- Adhere to policies, procedures, and performance metrics
What You Need
- Bachelor’s degree in health sciences, psychology, social work, communications, or related field
- 2+ years of relevant experience (e.g., case management, discharge planning, referral relations, healthcare admissions, or outreach)
- Strong interpersonal and relationship-building skills
- Results-oriented mindset with experience meeting performance metrics
- Excellent written and verbal communication skills
- High attention to detail and organizational skills
- Proficiency in Salesforce and Google Suite/MS Office
- Authorized to work in the US with native or bilingual English proficiency
Compensation & Benefits
- Base salary: $52,500 – $60,000 per year (final offer based on location and experience)
