Schedule
- Full-time, remote (U.S.-based)
- Must hold RN license in Texas, Florida, or a multi-state compact jurisdiction
- Minimal travel (up to 10%)
- Salary: $80,000–$90,000/year + performance-based bonuses
What You’ll Do
- Perform comprehensive assessments of behavioral and medical health needs
- Create and manage personalized care plans with patients, caregivers, and healthcare providers
- Remove barriers to care and ensure medication/treatment adherence
- Conduct medication reconciliation for safe care transitions
- Educate patients on health conditions and self-management strategies
- Advocate for timely access to medical and behavioral services
- Collaborate with an interdisciplinary team to enhance care outcomes
- Monitor progress, adjust care plans, and document all interactions
- Participate in daily huddles, team meetings, and quality improvement initiatives
What You Need
- Current RN license in TX, FL, or multi-state compact
- 3+ years behavioral health or medical case management experience
- Strong knowledge of behavioral health conditions, chronic diseases, and evidence-based interventions
- Excellent assessment, communication, and care coordination skills
- Proficiency in EHR systems and healthcare technology
- Commitment to patient-centered care and advocacy
Preferred
- CCM or similar certification
- Experience with diverse populations and community resources
- Bilingual in Spanish-English or Haitian Creole-English
