Solace is building a personal support layer for patients navigating a broken healthcare system. As a Registered Nurse (RN) Healthcare Advocate, youāll work directly with Medicare patientsāguiding them through health challenges, addressing social determinants of health (SDOH), and improving outcomes through empathy, action, and expert care navigation.

ā¢ Build meaningful relationships with Medicare patients through trust and advocacy
ā¢ Identify and prioritize patient needs, building holistic care plans

ā¢ Navigate patients through complex systems with clarity and compassion
ā¢ Use Solaceās proprietary tech tools to manage cases and support outcomes

šÆ Must-Have Traits
ā¢ Active RN license in good standing
ā¢ 3+ years of experience in patient advocacy, case management, or care navigation
ā¢ Strong clinical background paired with organizational excellence
ā¢ Experience working with Medicare populations and diverse communities
ā¢ Tech-savvy and fast at learning new tools
ā¢ Deep empathy and a strong sense of ownership
ā¢ Clear, consistent communicatorāespecially under pressure
ā¢ Comfortable giving feedback to improve systems and outcomes
