CareCentrix is hiring a remote Social Services Coordinator to support patients during critical care transitions. You’ll partner with nurse coaches to connect patients with vital resources—everything from Medicaid applications to meal support and transportation—ensuring they have what they need to heal at home. 📋 What You’ll Own
• Coordinate post-discharge support for patients and caregivers
• Submit Medicaid and reduced-cost drug applications as needed
• Match patients with resources like transportation, meal programs, and bill support
• Collaborate with nurse coaches and healthcare partners to ensure continuity of care
• Train internal teams on benefit access and community resources
• Engage directly with Health Plans to track eligibility and maximize patient coverage
• Maintain compliance with CareCentrix standards and HIPAA
🎯 Must-Have Traits
• Degree in Social Work or related field
• 5+ years of experience in community resource or Health Plan benefit management
• Proficient in Microsoft Office and EMRs, with ability to navigate multiple systems
• Deep knowledge of community resources and patient benefit coordination
• Strong organizational skills and compassion-driven communication
