Elevance Health is hiring Outreach Care Specialists to support its whole-health mission by ensuring members follow treatment plans, access care, and reduce risks. This role is primarily remote, with periodic in-person onboarding and training. You’ll help bridge gaps in care and support members in improving their overall health and well-being.

📋 What You’ll Own
• Support members with treatment plan follow-through
• Schedule follow-up care or program enrollment
• Call members to assess care compliance and identify challenges
• Suggest care plan changes and coordinate additional services
• Refer members to local, state, or federally funded programs
• Document progress, update case notes, and prepare reports
• Collaborate with care managers and provider networks
• Coach members on reducing health risks and improving outcomes
🎯 Must-Have Traits
• High school diploma or GED required
• Minimum 1 year related experience (healthcare, human services, etc.)
• Strong communication, problem-solving, and interpersonal skills
• Proficiency in documenting case notes and handling sensitive information
• Comfortable working independently in a structured virtual setting
• Preferred: Degree in social work, human services, or healthcare-related field
• Ideal: Experience as Community Case Manager, Health Worker, CNA, MA, or with mental health/substance use care

• Mostly virtual with required in-person training/onboarding
• Candidates must reside in one of the following: IL, IA, KS, LA, MN, MO, TN, TX, WI, NC
• Reliable internet connection and secure workspace
• Compliance with health protocols may be required (including vaccinations for some roles)
