CareBridge Health, part of the Elevance Health family, is looking for a mission-driven Outreach Care Specialist to support individuals in home and community-based care. This 100% virtual role allows you to work remotely while helping people access critical services that improve their independence, health, and overall quality of life.

📋 What You’ll Own
• Coordinate follow-up care by scheduling appointments and enrolling members in care programs
• Monitor treatment plan compliance via calls or visits
• Identify and resolve barriers to member care
• Recommend treatment modifications and flag additional service needs
• Refer members to local, state, or federally funded programs
• Coach members on risk reduction and health improvement strategies
• Track and document case updates and compliance reports
• Maintain strong working relationships with community agencies
🎯 Must-Have Traits
• High School diploma or GED (required)
• At least 1 year of experience in care coordination, healthcare, or related field
• Strong written, verbal, and interpersonal communication
• Able to work independently and manage caseload efficiently
• Detail-oriented and proactive in identifying care solutions
• Must reside in Tennessee
🛠️ Preferred Skills
• Certified Nurse Assistant (CNA) or Certified Medical Assistant (CMA) a plus
• Bachelor’s degree in a related field
• Familiarity with healthcare compliance (FMLA, ADA, HIPAA)
• Bilingual or multilingual skills (a plus for CareBridge)
💻 Remote Requirements
• Must live in Tennessee
• Reliable internet and ability to attend virtual onboarding
• Comfortable working full-time from home with minimal supervision
