Charlie Health is expanding access to life-saving mental health care—and as a Continuing Care Coordinator, you’ll be a vital bridge between our clients and the external providers who help continue their care journey. This role blends relationship-building with administrative precision: you’ll deliver treatment updates to referring sources and create thoughtful, clinically sound discharge plans that help our clients transition smoothly to their next step.

• Pay Range: $52,500 – $60,000 annually
• Employment Type: Full-Time, Exempt
• Remote: Yes (U.S. only)
• Bonus Eligible: Yes (potential additional compensation or incentives)
📋 What You’ll Own
• Deliver treatment updates and respond to inquiries from external referral sources
• Develop and manage discharge plans for clients completing treatment
• Connect clients to appropriate aftercare resources and confirm placement
• Maintain accurate and timely notes in internal systems like Salesforce
• Use and update provider databases to identify referral options
• Communicate across preferred channels with external providers
• Collaborate with internal clinical, admissions, and outreach teams
• Ensure all outgoing referrals have the necessary clinical materials
🎯 Must-Have Traits
• Bachelor’s degree in Health Sciences, Psychology, Social Work, Communications, or related field
• 2+ years of relevant healthcare experience (discharge planning, case management, outreach, etc.)
• Excellent verbal and written communication skills
• Organized and detail-oriented, with a results-driven mindset
• Strong interpersonal skills and ability to build trust with external partners
• Proficiency in Google Suite, Microsoft Office, and Salesforce
• Authorized to work in the U.S. with native or bilingual English fluency
