📋 What You’ll Own
• Assign and update patient visits using HomeCare HomeBase (HCHB) EMR
• Monitor workflow queues for timely visit completion and documentation
• Triage incoming requests based on urgency and patient care needs
• Work directly with field clinicians to support scheduling changes and updates
• Respond to scheduling tickets and phone support needs with accuracy and speed
• Escalate complex or time-sensitive issues to leadership
• Maintain productivity benchmarks and attend required meetings/trainings
• Work independently and stay organized in a remote environment
• Consistently show up and deliver—even on weekends or holidays
🎯 Must-Have Traits
• High school diploma or equivalent
• 1+ year in scheduling, healthcare support, or home health/hospice roles
• Strong preference for HCHB (HomeCare HomeBase) experience
• Impeccable attention to detail and schedule management
• Strong problem-solving skills under pressure
• Clear, professional communication—written and verbal
• High accuracy with data entry, patient info, and staff assignment updates
• Comfortable juggling multiple systems and tasks at once
• Familiarity with medical terminology
• Proficiency in Microsoft Office (Word, Excel, Teams, Outlook)
• Excellent phone etiquette and follow-through
• Must meet CareXM tech setup requirements
