✅ Position Highlights
• 💰 $80,000–$90,000 salary range
• 🕘 Full-time, Monday–Friday schedule
• 🏠 100% Remote (must hold RN licensure in a compact state: TX, FL, etc.)

🩹 What You’ll Own
• Conduct behavioral + medical assessments via telehealth
• Build personalized care plans with patients, caregivers, and clinicians
• Coordinate smooth transitions in care from hospital to home
• Perform comprehensive medication reconciliation
• Educate patients on conditions, meds, and care strategies
• Collaborate with a multidisciplinary team (psychiatrists, NPs, social workers, etc.)
• Track progress, update care plans, and advocate fiercely for patients
• Maintain clear, accurate documentation in the EHR
• Participate in team huddles, case conferences, and quality initiatives
🎯 What You’ll Bring
• Active RN license in TX, FL, or other compact states (required)
• 3+ years in behavioral or medical case management (required)
• Strong knowledge of mental health and chronic medical conditions
• Comfort with hospital discharges, care transitions, and preventing readmissions
• Excellent communication, empathy, and organizational skills
• Tech fluency with EHRs and remote collaboration tools
• Patient-first mindset and sharp clinical judgment

• Certified Case Manager (CCM) or similar credential
• Bilingual (Spanish-English or Haitian Creole-English)
• Familiarity with community resources and culturally responsive care
• Prior experience in outpatient, hospital, or community health settings
