🧾 About the Role
Vesta Healthcare is hiring a full-time Clinical Assistant to support members enrolled in Medicare’s Chronic Care Management (CCM) program. You’ll work from home, making outbound calls to patients and caregivers, coordinating care services, and ensuring that every member receives personalized attention that keeps them healthy at home.

• Pay: $20/hour
• Full-time (Monday–Friday, 8am–5pm ET, 9am–6pm ET, or 10am–7pm ET)
• Remote role with one weekend shift per month depending on team
• Dedicated to long-term care coordination and member outreach
• Company-provided laptop and remote setup reimbursement
📋 What You’ll Own
• Contact 20–30 members per day for care coordination, prescription refills, referrals, DME orders, and transportation
• Conduct telephonic outreach to patients, caregivers, and care teams
• Coordinate care in partnership with supervising providers
• Document notes and track workflows in clinical platforms
• Ensure accuracy through quality audits and reporting
• Support team operations with additional administrative tasks as needed
🎯 Must-Have Traits
• Certified or Registered Medical Assistant (required)
• At least 1 year of experience in a healthcare setting (PCP, clinic, or telehealth)
• Knowledge of referrals, insurance coordination, and CCM workflows
• Strong communication and customer service mindset
• Comfortable working independently and remotely
• Must be authorized to work in the U.S.
📜 Preferred Qualifications
• Fluency in English + Spanish, Mandarin, Cantonese, or Russian (reading/writing/speaking)
